The Northern Devon Healthcare NHS Trust has organised its integrated community health and social care teams around clusters of GP practices.
With you, these teams are now supporting 6000 Devonians in their own homes by providing proactive health and social care interventions.
This dedicated web area will contain news and updates on our progress in transforming community services as well as the policies and standards of medical care to support those of you that provide medical cover to our community hospitals.
We have put some information in the menus which we hope will help you provide high quality GP medical cover to the Community hospitals in East and North Devon.
